Nyrsio Cymunedol

Our community nursing services are provided by teams of community based registered nurses and health care support workers.  The District, Specialist Nurses’ team is  led by qualified district nurses.  Moreover, the district nursing team provides both assessments and care management.  This provision is intended for people that need unscheduled, long or short term care, as well as palliative and terminal care. Principally, this is provided within the patient’s own home (which includes residential care settings) or where it is most appropriate, based on service provision/clinical judgement.  Find out more yma..

Of course, the District & Specialist Nurses service promotes healthier lifestyles, physical, psychological and social wellbeing.  Furthermore, we encourage and support people with disability and chronic conditions to live as independently as possible.
