We are a team of social workers and care management assistants who work with adults over the age of 18 with a physical disability, older people who are 65 years or over with care and support needs and carers.
We provide information, advice and assistance, undertake assessments, develop care and support plans. We work alongside the voluntary sector to provide preventative services. All the work we do is done on a co-production approach which means that the person and their well-being is at the heart of what we do. We support people to maintain their independence and build on their own resources.
We work across the Tywi, Taf Together geographic area on a multi-agency basis which involves working with colleagues such as GPs, Occupational Therapists, Physiotherapists, Specialist nurses, Mental health practitioners and Psychologists to ensure that individuals receive a collaborative and seamless service. Referrals can be made through our Information, Assistance and Advice (IAA) service through Delta Wellbeing on 0300 333 2222. We accept self-referrals or from anyone where the consent has been given by the person who appears to have care and support needs.
