The learning disability primary care liaison service  is for people with a learning disability to support their access to an annual health check within their GP Surgery. 

The service has two experienced part time learning disability nurses covering Pembrokeshire, Ceredigion and Carmarthenshire.  In addition to this, there are also 3 people with a learning disability, employed by HDUHB to support the facilitation of training and education to Primary Care professionals.

We want to make sure that people with learning disabilities have good experiences of accessing healthcare.  We will also support Primary Care services to improve the uptake and quality of their annual health checks in line with Improvement Cymru’s agenda to improve the health inequalities of people with a learning disability.

Our Services

  • The main focus of our role is to provide training to primary care staff about the health needs of people with a learning disability.
  • To give advice to people with a learning disability, their families and carers about what to expect from/during and after their Annual Health Check
  • To liaise with the patient/carers and multiple agencies to ensure that Health Action Plans are developed and implemented.
  • We also give support and advice about reasonable adjustments in line with the Equality Act.
  • We can support with communication between the patient, carers, primary care staff and local learning disability services i.e. Community Teams.

Other Types of Support

  • ‘Sit in’ on Annual Health Checks to offer advice and sign-posting
  • Helping to update LD registers held within practices
  • Support patients to prepare for the health check e.g. completion of the Pre-health check questionnaire
  • We can adapt to the needs of the practice
