The learning disability primary care liaison service is for people with a learning disability to support their access to an annual health check within their GP Surgery.
The service has two experienced part time learning disability nurses covering Pembrokeshire, Ceredigion and Carmarthenshire. In addition to this, there are also 3 people with a learning disability, employed by HDUHB to support the facilitation of training and education to Primary Care professionals.
We want to make sure that people with learning disabilities have good experiences of accessing healthcare. We will also support Primary Care services to improve the uptake and quality of their annual health checks in line with Improvement Cymru’s agenda to improve the health inequalities of people with a learning disability.
