How can I access a Social Prescriber? <\/strong><\/span><\/p>

You can fill in a self-referral form by clicking on the link here,<\/a><\/strong> and returning it to your GP surgery or emailing the completed form to SocialPrescribing@carmarthenshire.gov.uk<\/a><\/strong> . You can also speak to your GP, surgery staff or health care professional and they can make the referral on your behalf.<\/span><\/p>

After a referral has been made you will be contacted by the social prescriber at your surgery and invited to have a face to face or telephone appointment to discuss your issues confidentially. Together you will explore what is important to you and you will be supported to access local services and activities to improve your health-wellbeing. Social Prescribers will not give you medical advice.<\/span><\/p>

In order to meet the referral criteria, you must be aged 18+ and not be in crisis.\u00a0 If you are in crisis, please contact your GP as soon as possible.<\/span><\/p>\t\t\t\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Sometimes patients visit their GP for reasons other than medical issues as a result of not knowing where to get support for their wider social issues, which have a significant, negative impact on their health. Social Prescribing is an approach that recognises the connection between our physical health and our social, emotional, and mental wellbeing. The social prescribing model in Carmarthenshire is holistic, person-centred and integrates with statutory services across sectors to provide a proactive approach to improving health and wellbeing. The social prescribing service aims to enable people to maximise their own potential, promoting self-management and supporting individuals to take greater control of their own health through enabling individuals to connect with their local community.<\/p>\n

Who are Social Prescribers? <\/strong><\/p>\n

Social Prescribers in Tywi Taf are wellbeing advisors, based in GP practices, who support patients with non-medical issues and help them to access appropriate support within the local community, for example around volunteering, employment, benefits, housing, debt and physical activity.<\/p>\t\t\t\t\t<\/div>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t
