The Community Resource Team (CRT) has been running since April 2009 and consists of a multi-professional team working very closely with Primary and Secondary Care.

We promote people to stay independent and to maintain choice and control over their lives.

The Community Resource Team brings together professionals from health, social care and the third sector to provide care to people so they can remain in their own homes, at the same time providing support to their families and carers.

The Team offers an improved service that ensures that people receive the right intervention, at the right time, from the right professional.

 

There are staff on duty every day who can respond to urgent situations to provide care closer to home and we work very closely with the GPs to prevent hospital admissions.

We aim to support people, families and carers to live as well as possible by exploring the issues and problems that are affecting their daily health and wellbeing.  This approach allows a more co-ordinated, integrated way of working addressing what matters to the person.
